New Patient Forms

If you are a new patient to Transitions, please follow this link to our patient registration forms. Please print and fill out all of the forms  prior to your visit.

What to expect at the first visit

You should plan on the initial evaluation / first visit to be 45 minutes. Please arrive early to check in with the office. Wear comfortable clothes that  allow access to your involved area. The first visit will consist of a review of how you are managing and a quick history of your injury and current functional levels. The therapist will perform an orthopedic examination and spend time educating you about the findings and proposed plan of care.

Please note that overflow parking at the Essex Clinic is available at the Calvary Church

Performing Arts Playing Related Pain

Playing Related Musculoskeletal Disorders (PRMD) are prevalent in musician's.
